This is an important message for previous patients of Dr. Amuthan who have NOT yet completed updated enrolment forms for her replacement, Dr. Long Kin (Calvin) Poon.

Our clinic has been reaching out to previous patients of Dr. Amuthan regarding enrolment forms to transition them to her replacement, Dr. Long Kin (Calvin) Poon. We have attempted to contact these patients by phone and email since December 2023. 

If you have not received these messages, please contact our office to update your contact information and to arrange to have our clinic policies and enrolment forms sent to you for completion by May 31, 2024. 


***Please note - if you have NOT completed and submitted enrolment forms to our clinic by May 31, 2024, we will assume that you are NO longer interested in continuing under our care. As such, your patient profile will be closed, and you will no longer be a patient at Towne Centre Medical Clinic.
