This is an important message for previous patients of Dr. Sjodin, who did not complete the enrolment forms for her replacement, Dr. Amanda Yee:

Our clinic has contacted all previous patients of Dr. Sjodin on several occasions, beginning at the time of her retirement in December 2022, regarding the enrolment forms needed to transition care to her replacement, Dr. Amanda Yee.

As stated in the email notices sent out at that time, if you have not completed and submitted the enrollment forms to our clinic by May 31, 2023, the time to do so has since expired, and her practice is now closed. As such, your patient profile has been closed, and you are no longer a patient here at Towne Centre Medical Clinic.
